HCV SUPPORTIVE HOUSING COORDINATOR

REPORTS TO: DIRECTOR OF FSS & SPECIAL PROGRAMS

Salary Range:$50,000.00 To $55,000.00 Annually

THIS IS AN EXEMPT POSITION

POSITION SUMMARY: Under the general direction of the Director of FSS & Special Programs, the HCV Supportive Housing Coordinator is responsible for the development, coordination, and implementation of a comprehensive supportive services model designed to serve vulnerable populations assisted by the Housing Choice Voucher (HCV) program.

This position is responsible for assessing participants' housing stability and human service needs, while building and maintaining strategic partnerships with community providers to ensure access to appropriate supportive services. The Coordinator develops and implements program strategies that promote self-sufficiency, long-term housing stability, and improved overall well-being, with a focus on serving individuals and families with complex needs.

The HCV Supportive Housing Coordinator serves as a key liaison between participants, service providers, landlords, and internal staff, ensuring a collaborative and integrated approach to service delivery. At all times, the Coordinator maintains a respectful, professional, and supportive attitude toward residents, participants, staff, and community stakeholders while upholding the mission and values of the Lake County Housing Authority.

ESSENTIAL FUNCTIONS:

  1. Provides comprehensive, wraparound case management and supportive services to Lake County Housing Authority (LCHA) program participants and residents.
  2. Assists participants in identifying and securing permanent housing through proactive outreach and relationship-building with landlords, property managers, realtors, and supportive housing providers.
  3. Educates participants on available community resources to promote housing stability and self-sufficiency, and coordinates referrals based on individualized needs.
  4. Collaborates with internal staff and external partner agencies to expand resource networks, facilitate referrals, and ensure effective follow-up to support participant stability and overall well-being.
  5. Effectively manages an assigned caseload of approximately 40-45 participants monthly, ensuring timely and responsive service delivery.
  6. Maintains accurate, complete, and up-to-date client records in accordance with program policies, procedures, and regulatory requirements.
  7. Demonstrates and promotes LCHA's mission, vision, and values while fostering a high level of cross-departmental collaboration to support a "Complete Care" service approach.
  8. Performs Housing Choice Voucher (HCV) program functions in alignment with HUD regulations and SEMAP performance indicators, including:
  • Assisting with accurate income verification, annual/interim reexaminations, and rent calculations to ensure correct tenant rent portions and housing assistance payments (HAP).
  • Supporting rent reasonableness and payment standard compliance in coordination with HCV staff.
  • Ensuring timely and accurate documentation to support SEMAP indicators related to reexaminations, rent determinations, and file integrity.
  1. Ensures compliance with applicable HUD regulations and program requirements, including but not limited to Mainstream (MS5), Emergency Housing Vouchers (EHV), and Fair Share Vouchers, maintaining current knowledge of regulatory changes and program guidance.
  2. Performs other duties as assigned.

RELATED DUTIES AND RESPONSIBILITIES:

  1. Provides appropriate referrals and supportive services tailored to each participant's individual needs.
  2. Advocates on behalf of participants to ensure access to services and resources necessary to achieve housing stability and overall well-being.
  3. Performs other reasonably related duties as assigned by the immediate supervisor or management team.

PARTNERSHIP DEVELOPMENT & COMMUNITY COORDINATION:

  1. Build and maintain strong partnerships with healthcare providers, behavioral health agencies, social service organizations, workforce development programs, and local governments.
  2. Formalize partnerships through MOUs or service agreements where appropriate.
  3. Coordinate case conferencing and multidisciplinary team approaches for high-need households.

DATA TRACKING, COMPLIANCE & REPORTING:

  1. Maintain accurate and timely case records in Yardi and other required systems.
  2. Track outcomes related to housing stability, service utilization, and participant progress.
  3. Ensure compliance with HUD regulations, MTW requirements, and agency policies.
  4. Prepare reports for leadership, HUD, and Board as needed.
  5. Support reporting, quality control, and audit readiness by ensuring completeness and accuracy of electronic records.
  6. Manage and maintain client data within the Homeless Management Information System (HMIS), including timely data entry, document management, data quality monitoring, and compliance with agency, funder, and confidentiality standards.

MINIMUM QUALIFICATIONS:

  1. Bachelor's degree in Social Work, Human Services, Public Administration, or a related field (Master's preferred).
  2. Minimum of 3-5 years of experience in supportive housing, case management, or human services.
  3. Experience working with vulnerable populations, including individuals experiencing homelessness, persons with disabilities, or those with behavioral health needs.
  4. Knowledge of HUD programs, including HCV, Continuum of Care, or similar programs preferred.
  5. Strong understanding of community resources and service systems.
  6. Excellent communication, organizational, and problem-solving skills.
  7. Ability to build partnerships and work collaboratively across sectors.
  8. Proficiency in data management systems (Yardi experience preferred).
